"At least one person has to be with her and watch her while showering without belt": Well IF someone is wearing a belt for anti-masturbation purposes, it makes sense not to leave a loophole here. "watch" could be interpreted loosely in this case: There's no reason to stare all the time. Just being around to make sure there are no unreasonable delays with her hygiene would be sufficient.
"She can only have one orgasm every quarter year": Not sure what the intention of this is. Like @youdontknowme said: If she's supposed not to masturbate, then it's too often. And if only excessive masturbation is supposed to be limited, it seems too strict.
"Cell phone is checked every week for any erotic pictures, games etc., cell phone trash is also checked": Hard no from me, this would be a major privacy intrusion, as if you would be looking over her shoulder ALL the time.
"No erotic videos/novels, porn or porn related images/videos etc.": Would not be enforceable anyway without the cellphone checks, which I'm against, so also no, if only for practical reasons.
"Doors stay open, no matter if bathroom or bedroom!": Also a big privacy violation here, so no. Sounds like jail-level surveillance. What is the intention here? You don't trust the belt to do its job?
"When orgasm is allowed once per quarter year she is observed while masturbating": Disturbing idea. Not only because of the extreme privacy violation, but as others already mentioned, it seems to imply sexual motives on your side.
