Ah. Over here is it mostly communal men's and women's changing areas and some gender-neutral individual stalls at pools, from the places I have seen. Gyms usually just have the gendered areas, but with a lot of people showering at home, most gym users only strip down to their underwear.
The toilet cubicle certainly explains how you can take off the belt in private, but is an unbelted toilet stall safe? I guess toilet masturbation might mostly be a guy thing, for... ahem... waste disposal reasons.

Honestly, with how much of a role opportunity and violent impulses play in those sorts of crimes, trying to get the key seems like it would be a fairly unusual route for an attacker to take; responding to the surprise of a belt with violence that does not involve penetration seems to be more likely to me.. But I guess it is easy for me to speculate, as an unremarkable male I just do not fit the typical profile of potential victims, so I cannot really give a qualified opinion on potential threats.
