With reports on the internet, fantasy versus reality may often be difficult to distinguish. Yes, some very open cage designs might work for 24/7 wear over a month or so if you are lucky with the way your body works, but anything beyond that is probably irresponsible in any case. The longest periods of uninterrupted wear were by circumcised males in fairly open piercing-only devices. Some others have worn ball-trap cages with the keyhole rendered unusable for months at a time, but the case I know about was a guy who was able to pull out of his cage and clean up that way.
Keeping a full belt clean is much more difficult to manage; a waistband that is tight enough to be comfortable and secure at the same time is going to be a challenge to wash properly under, and you definitely need to be able to reach all the boy parts with water, soap and scrubbing motions - which will probably compromise the security of the belt at least a little. BehindBarz has some rather open tubes ("inner cage"s), those probably help, but the cup ("outer cage") needs to be equally open or, better, removed for hygiene breaks. There are options from the manufacturer to make the cup removable without unlocking the belt, or even to make the cup lock with a different key without being able to unlock the belt and tube. Maybe that is an option for you to make sure you are still, in a way, locked while taking a hygiene break.
And even then, I would still consider it irresponsible to not at least take the belt off once a month to check for skin damage and give everything a good deep clean.
And keep in mind, circumcised penises are easier to keep clean while secured from prying hands, but that is a pretty major step to undertake just to reduce the frequency of unlocks a bit.
