well, there should always be at least a small barrier and a mechanism to make sure that the key holder always knows when the emergency key is used. this can be done with seals (envelope or similar), or with number codes on ... something that only the key holder knows.
Emergency key
youdontknowme hey better have a good explanation why the belt had to come off
"By accident".
HHelen double tamper seals on the belt and the key with regular inspection of both should make that option extremely dubious. "Yeah, somehow that seal on the key container got damaged, which swept the key into the lock, turned it and removed the belt."
But yeah, designing a good seal is somewhat of a balance between making it easy enough to break intentionally in an emergency, but hard enough to break itself inadvertently.

That's why I use both key and seal.

Owl It is like those emergency room appointments where some dude was just innocently cleaning his apartment naked and unexpectedly fell off the ladder and butt-first onto a coke bottle

well, when using a seal, i would proceed in such a way that if it breaks by mistake, i immediately contact the key holder and explain what happened, then one can usually prevent misunderstandings
If anyone is looking for a remote / Bluetooth solution for locking keys I just got one of these.
www.amazon.com/dp/B09C7V9798?psc=1&ref=ppx_yo2ov_dt_b_product_details. Currently about $45.00 USD. (subject to change with no notice )
The keyholder sets it up, and adds the person in chastity as a friend. I've had it less than a week, so I can't tell about long term stuff; however, it appears relatively simple and easy to use.
this is a great idea and also not expensive
Thank you. It seems interesting but the ad IMO also leaves open questions. To delve into more details, I outsourced that product into an own thread, and put my question (and worries) there. See: https://unwedchastity.org/d/744-imagic-bluetooth-key-lock-box-experience-comments/2

Do you think we have bolts in urgency boxes?
Obviously tools can be achieved, but medical staff losses time.
A belt in a hospital is always a problem it should go out.
